Items Required from New Students:

· Non-refundable $50.00 application fee and $100.00 registration fee
· Birth Certificate
· Baptism Certificate (if Catholic)
· Immunization Record
· Health Records (signed by Doctor or Health Care Provider)
· Past records from transfer school (recent report card)
· Standardized Test Scores
· Recommendation letter from the teacher or the Principal of the last school.

*Other registration requirements will be completed when the above registration documents have been turned in to the school office and the principal has reviewed the application. If you should need any other forms or have any questions, do not hesitate to call the school.
